Job summary

Qualifications

• Must be a graduate from an accredited school of Registered Nursing.
• California RN license is required.
• BART or BLS at time of hire with commitment to obtain BART within six months.
• Minimum of 3 years inpatient RN care management experience in an acute care hospital or equivalent with discharge planning expertise.
• Utilization review/discharge planning experience.
• Proficiency in MS Office and data entry; strong typing skills.
• Strong interpersonal, leadership, negotiation, and hospital operations knowledge.

Responsibilities

• Facilitate the patient’s hospitalization from pre-admission through discharge.
• Coordinate with physicians, nurses, social workers, and other healthcare providers to expedite appropriate care.
• Apply clinical expertise and utilization criteria to discharge planning and resource use.
• Communicate with administration and healthcare team to facilitate patient care.
