Job summary

Hireology is seeking a per diem Physical Therapist for home health services in New Jersey. The role involves evaluating patient function, developing and adjusting treatment plans, and collaborating with physicians to ensure quality care across settings.

The PT will perform assessments, provide therapeutic interventions, document progress, and educate families while adhering to regulations and organizational policies.

Qualifications

• Active PT license in NJ
• 2 years PT experience in relevant population
• CPR certification within 60 days

Responsibilities

• Assist in evaluating patient function and equipment needs and develop plan of care.
• Complete initial and ongoing comprehensive assessments.
• Provide interventions per physician plan of care and professional standards.
• Communicate with family and caregivers to support progress and discharge planning.
• Educate patient and family to promote progress toward goals.
• Document patient care and responses to treatment clearly and timely.
• Supervise PT assistants as assigned.
