Job summary

Beyond Home Health Care in New Jersey seeks a licensed Physical Therapist to deliver in-home and community-based services to adults with developmental disabilities. You will evaluate mobility, strength, balance, gait, and functional movement, and implement person-centered interventions aligned with NJ DDD guidelines.

The role involves one-on-one therapy in homes and communities, documentation, collaboration with supports Coordinators and families, caregiver training, and adherence to DDD

Qualifications

• Current New Jersey license as a Physical Therapist (PT) issued by the NJ State Board of Physical Therapy Examiners.
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one (1) year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver's license, and current auto insurance.
• Ability to pass DDD-required background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy services in the participant's home and community settings.
• Document services in compliance with DDD documentation standards, including session notes, progress notes, and outcome measures, submitted within required timeframes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce therapeutic goals between sessions.
• Adhere to all DDD policies and HIPAA confidentiality standards.
• Maintain accurate billing records consistent with DDD Fee-for-Service rates and authorizations.
• Complete required DDD-mandated trainings (Pre-Service, Danielle's Law, Stephen Komninos' Law, etc.).
