Job summary

JAG Physical Therapy is seeking a Per Diem Physical Therapist to cover as-needed needs in New Jersey. The clinician will evaluate patients, develop and implement plans of care, and collaborate with the interdisciplinary team to maximize functional recovery.

Responsibilities include documenting progress, educating patients on home programs, and contributing to a high-energy, positive clinical environment. NJ licensure and CEU compliance are required.

Qualifications

• Licensed Physical Therapist in NJ with current credentials.
• Degree required; Doctorate preferred for advanced practice.
• Experience with EMR systems and strong communication.

Responsibilities

• Conduct initial evaluations and ongoing assessments of patients.
• Develop and adjust treatment plans to meet goals.
• Record daily notes, progress, and discharge plans.
• Educate patients and families on home programs and safety.
• Use therapeutic exercises and modalities as prescribed.
• Collaborate with the care team to optimize patient outcomes.
