Job summary

Beyond Home Health Care is a NJ DDD-approved provider offering in-home physical therapy services across New Jersey. We seek a licensed PT to evaluate mobility, strength, balance, gait, and functional movement, delivering person-centered interventions to promote independence and community participation.

The role includes home-based evaluations, one-on-one therapy, and collaboration with coordinators and families. NJ license, CAPTE-certified degree, CPR/BLS, and background checks required.

Qualifications

• Current NJ PT license is required.
• Graduate from CAPTE-accredited program (DPT or equivalent).
• CPR/BLS certification required.
• Clean background check & fingerprints per NJ DDD.
• Reliable transportation and NJ driver’s license.

Responsibilities

• Conduct comprehensive PT evaluations and develop individualized treatment plans.
• Deliver in-home and community-based PT services.
• Document services per DDD standards and timelines.
• Collaborate with coordinators, families and rehab team for continuity of care.
• Provide caregiver training and home exercise programs.
• Adhere to DDD policies and HIPAA standards.
