Qualifications

• Two years of physical therapy experience in the last five years.
• Current licensure as a Physical Therapist in NJ.
• Graduate of an accredited physical therapy education program.
• CPR certification (to be achieved within 60 days)

Responsibilities

• Assist evaluating patient function and plan of care/treatment with goals, frequency, and duration.
• Complete initial and ongoing patient assessments.
• Provide interventions and treatment per physician plan and PT standards.
• Re-evaluate and revise services as patient status changes.
• Educate patient and family to promote progress and discharge planning.
• Document all care and patient responses per standards.
