Job summary

Beyond Home Health Care is seeking a licensed Physical Therapist to provide in-home and community PT services across New Jersey. The PT will evaluate mobility, strength, balance, gait, and functional movement, delivering person-centered interventions to maximize independence according to NJ DDD guidelines.

Responsibilities include comprehensive evaluations, treatment planning, and collaboration with families and coordinators. Competitive hourly pay and flexible scheduling are offered.

Qualifications

• Active CPR/BLS certification.
• New Jersey PT license required.
• Graduate of a CAPTE-accredited PT program (DPT or equivalent).
• Minimum of 1 year of clinical PT experience preferred; new grads considered.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver’s license, and current auto insurance.
• Ability to pass DDD background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy services in home and community settings.
• Document services per DDD standards (session notes, progress notes, outcomes) within required timeframes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Adhere to DDD policies and HIPAA confidentiality standards.
• Maintain billing records consistent with DDD rates and authorizations.
