Job summary

VNA Health Group seeks a compassionate per diem Physical Therapist to join our homecare team in Hudson County. You will evaluate and provide skilled PT services to patients in their homes, helping them maintain mobility and independence.

The role requires NJ PT licensure, 2–3 years of clinical experience (homecare a plus), strong communication skills, and reliable transportation. Per diem schedule with comprehensive benefits through VNA Health Group.

Qualifications

NJ Physical Therapy license required.
2–3 years of clinical experience preferred; homecare or community-based experience a plus.
Strong interpersonal and communication skills.
Excellent computer skills, including documentation via tablet or phone.
Valid NJ driver’s license and personal vehicle.

Responsibilities

Deliver skilled, one-on-one physical therapy care in patients' homes.
Partner with physicians and the interdisciplinary team to design effective care plans.
Promote independence and safety through personalized rehabilitation.
Make a meaningful impact on the lives of patients and their families.
Assess and evaluate patients for physical therapy needs.
Develop, implement, and revise individualized treatment plans in coordination with physicians.
Document clinical status, changes, and outcomes.
