Job summary

Beyond Home Health Care is a licensed physical therapy provider delivering in-home services across New Jersey. We seek a licensed Physical Therapist to evaluate mobility, strength, balance, gait, and functional movement, and to deliver person-centered interventions to maximize independence and participation.

The role requires NJ PT licensure, CAPTE-aligned training, and ability to work in home and community settings with flexible scheduling and supportive leadership.

Qualifications

• NJ license as a Physical Therapist (PT) in good standing.
• Graduate of a CAPTE-accredited PT program (DPT or equivalent).
• Minimum of 1 year clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver’s license, and current auto insurance.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy services in home and community settings.
• Document services per DDD standards with session and progress notes.
• Collaborate with Support Coordinators, families, caregivers, and the care team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Adhere to DDD policies and HIPAA confidentiality standards.
• Maintain accurate billing records consistent with DDD rates and authorizations.
