Job summary

TRU Healthcare is seeking a Per Diem Occupational Therapist to provide home health services in Duluth, MN. You’ll assess and treat patients in their homes, focusing on ADLs, IADLs, safety, and independence, with a flexible, per-visit schedule.

Responsibilities

• Provide skilled occupational therapy services in patients' homes based on individualized treatment plans.
• Evaluate patients' functional abilities and develop strategies to improve independence in ADLs and IADLs.
• Address deficits in strength, coordination, balance, cognition, mobility, and daily activities.
• Educate patients on adaptive equipment and safety techniques for independence.
• Assess home environments and recommend fall prevention and accessibility improvements.
• Educate patients and caregivers on home exercise programs and safety strategies.
• Monitor progress and adjust treatment as needed; document accurately and timely.
• Complete accurate and timely clinical documentation.

Your Role as a Per Diem OT

As an Occupational Therapist with TRU Healthcare, you'll help patients regain the skills, confidence, and independence they need to safely navigate everyday life in their own homes.

• Provide skilled occupational therapy services in patients' homes based on individualized treatment plans.
• Evaluate patients' functional abilities and develop treatment strategies focused on improving independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Address functional limitations related to strength, coordination, balance, cognition, mobility, and other areas affecting daily life.
• Recommend and educate patients on adaptive equipment, compensatory strategies, and techniques to improve safety and independence.
• Assess home environments and provide recommendations for fall prevention, accessibility, and safe performance of daily activities.
• Educate patients and caregivers on home exercise programs, safety strategies, and techniques to support continued progress between visits.
• Monitor patient progress, update treatment approaches as needed, and communicate changes with the interdisciplinary care team.
• Complete accurate and timely clinical documentation.

What You'll Bring

• Current, valid Minnesota Occupational Therapist license.
• CPR/BLS certification.
• Previous home health experience preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work independently while collaborating effectively with the interdisciplinary care team.
• Ability to travel withinDuluth and surrounding areas.
• Reliable transportation and willingness to travel between patient visits. Mileage reimbursement provided.
• A genuine desire to help patients live more safely and independently.
