Job summary

TRU Healthcare is seeking a Per Diem Occupational Therapist to provide home health services in Duluth, MN. You’ll assess and treat patients in their homes, focusing on ADLs, IADLs, safety, and independence, with a flexible, per-visit schedule.

You’ll collaborate with the interdisciplinary team, complete timely documentation, travel within Duluth and surrounding areas, and enjoy mileage reimbursement. Minnesota OT license and CPR/BLS are required; home health experience preferred.

Qualifications

• Current, valid Minnesota Occupational Therapist license.
• CPR/BLS certification required.
• Home health experience preferred.
• Strong clinical, communication, and organizational skills.
• Ability to travel within Duluth and surrounding areas.

Responsibilities

• Provide skilled occupational therapy services in patients' homes based on individualized treatment plans.
• Evaluate patients' functional abilities and develop strategies to improve independence in ADLs and IADLs.
• Address deficits in strength, coordination, balance, cognition, mobility, and daily activities.
• Educate patients on adaptive equipment and safety techniques for independence.
• Assess home environments and recommend fall prevention and accessibility improvements.
• Educate patients and caregivers on home exercise programs and safety strategies.
• Monitor progress and adjust treatment as needed; document accurately and timely.
• Complete accurate and timely clinical documentation.
