Job summary

Qualifications

• Must hold a current Minnesota Physical Therapist license.
• Graduation from an accredited Physical Therapy program.
• Patient-centered mindset and clinical decision-making skills.
• Ability to work in home health settings with independence.
• Previous home health experience preferred.
• Valid driver's license and reliable transportation.
• Ability to collaborate with care teams.
• Strong assessment and treatment planning skills.

Responsibilities

• Provide one-on-one patient care in patients' homes.
• Evaluate and treat patients to improve mobility and independence.
• Coordinate with nurses, aides, physicians, and families.
• Educate patients and caregivers on exercises and safety.
• Maintain timely clinical documentation.
• Travel between patient visits.
