Qualifications

• Must hold a current Minnesota Physical Therapist license.
• Graduation from an accredited Physical Therapy program.

• Valid driver's license and reliable transportation.
• Ability to collaborate with care teams.
• Strong assessment and treatment planning skills.

Responsibilities

Your Impactful Role as a Physical Therapist

In this role, you will:

• Provide One-on-One Patient Care: Evaluate and treat patients in their homes, developing individualized treatment plans focused on mobility, function, independence, and quality of life.
• Enjoy Scheduling Flexibility: Manage your patient visits and accept work that aligns with your availability and the needs of our patients.
• Help Patients Reach Their Goals: Use your clinical expertise to help patients improve strength, balance, mobility, and functional independence.
• Collaborate with the Care Team: Work with nurses, occupational therapists, aides, physicians, patients, and families to coordinate comprehensive care.
• Educate Patients and Caregivers: Provide instruction on therapeutic exercises, mobility, safety, and strategies that support continued progress at home.
• Maintain Clinical Documentation: Complete timely, accurate documentation and communicate patient progress and changes in condition to the appropriate care team members.

What You'll Need to Excel

• Licensure: A current Minnesota Physical Therapist license.
