Job summary

ComplexCare Solutions seeks a Nurse Practitioner for a per diem role to conduct in-home and telehealth assessments for health plans nationwide. The provider will spend up to an hour with members, capturing a thorough view of needs, medical conditions, and management plans to keep patients at home and out of hospital settings.

The position requires ANCC or AANP NP board certification and an active NP license, with HIPAA compliance and CMS guidelines.

Qualifications

• Must have ANCC or AANP NP board certification and active NP license.
• Experience reviewing chronic conditions and coordinating care per CMS/DHHS requirements.
• Ability to work autonomously in a remote clinical environment and manage patient assessments.

Responsibilities

• Perform comprehensive member assessments in home/telehealth settings.
• Document and communicate assessment findings, recommendations, and follow-up needs.
• Address gaps in care and coordinate with PCPs and health plans as required.
