Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for per diem work to perform in-home and telehealth assessments across assigned zip codes in Adams County, CO. Providers will spend a full hour with members to capture a comprehensive view of needs and plan care accordingly.

Must hold an active NP license with ANCC/AANP certification and CMS/HIPAA familiarity. Remote modality and 25 slots per month (≈37 hours) are expected, with continued documentation and timely communication to PCPs.

Qualifications

• Active Nurse Practitioner license with ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine.
• Must meet CMS/HIPAA requirements and maintain documentation accuracy in medical records.

Responsibilities

• Conduct comprehensive home and telehealth assessments for members.
• Document all conditions and assessments in medical records per CMS and client requirements.
• Communicate findings and recommendations to PCPs/health plans and coordinate follow-up.
