Job summary
ComplexCare Solutions is seeking a Nurse Practitioner for per diem roles to deliver high-value home and virtual assessments for health plans nationwide. You will engage with members for comprehensive evaluations, document findings, and coordinate with physicians and health plans to improve clinical outcomes.
The role requires an active NP license, national certification, CPR, and the ability to work autonomously in a remote clinical environment.
Qualifications
- Active unencumbered license to practice nursing.
- ANCC or AANP board certification as a Nurse Practitioner or Clinical Nurse Specialist.
- Maintains current CPR certification.
- Vaccination requirements: Hepatitis B, Influenza, MMR, Pertussis/Tetanus/diphtheria (Tdap), Varicella, Tuberculosis.
- Home Health experience is a plus.
Responsibilities
- Conduct comprehensive assessments of members in home/virtual settings and document findings.
- Communicate results and care recommendations to PCPs and health plans.
- Identify and address gaps in care and opportunities to improve outcomes.
- Perform Point of Care testing and document results.
- Maintain HIPAA compliance and complete encounter documentation in a timely manner.
