Job summary

ComplexCare Solutions (CCS) is seeking a Nurse Practitioner for per diem work delivering in-home and telehealth assessments for health plans nationwide. You will spend time with each member to capture a comprehensive view of needs, risk factors, and care management opportunities to keep members at home and out of the hospital.

An active NP license, ANCC/AANP certification, and CPR are required; home health experience is a plus.

Qualifications

• Active, unencumbered NP license.
• ANCC or AANP board certification as NP.
• Current CPR certification.
• Knowledge of CMS/Medicaid guidelines is beneficial.
• Experience with home health preferred.

Responsibilities

• Conduct in-home and telehealth assessments with members.
• Document findings and care plans in the medical record.
• Coordinate with PCPs and health plans for timely follow-up.
• Identify gaps in care and provide evidence-based recommendations.
• Maintain HIPAA compliance and documentation quality.
