Job summary

ComplexCare Solutions is seeking a Nurse Practitioner per diem to perform in-home and telehealth assessments across designated ZIP codes in Marion County, OR. The role emphasizes comprehensive patient reviews, evidence-based treatment planning, and coordination with PCPs and health plans.

You will conduct POC testing, document encounters, and ensure CMS/HIPAA compliance while meeting patient needs in a remote clinical setting. A sign-on bonus of $3,500 is offered.

Qualifications

• Active unencumbered NP license in the US.
• ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine.
• Current CPR certification.
• Vaccination/testing per healthcare requirements (HBV, influenza, MMR, Tdap, varicella, TB).
• Home health experience is a plus.

Responsibilities

• Conduct comprehensive home assessments and document encounters.
• Ensure CMS/DHHS compliance and HIPAA adherence in all records.
• Communicate findings and treatment plans to PCP and health plan.
• Close gaps in care and provide guideline-based recommendations.
• Perform and document Point-of-Care testing; maintain records.
