Job summary
ComplexCare Solutions is seeking a Nurse Practitioner for per diem in-home assessments across Douglas County, OR. Providers will spend about one hour per member to capture comprehensive needs and determine home-based care requirements to prevent hospitalizations.
Responsibilities include documenting medical conditions, communicating with PCPs and health plans, and adhering to CMS/NCQA guidelines. Requires an active NP license and board certification with CPR on file; field experience and
Qualifications
- Must hold an active unencumbered nursing license.
- ANCC or AANP board certification as a Nurse Practitioner or Clinical Nurse Specialist in Family, Adult, Gerontology or Emergency Medicine.
- Maintains CPR certification.
- Proficient with computers, EHRs, secure messaging and cloud-based documentation.
Responsibilities
- Review assigned member population and document encounters thoroughly.
- Provide evidence-based treatment plans and timely follow-up with PCP and health plan.
- Escalate emergent or urgent situations per policy and professional judgment.
- Conduct, document and communicate results of Point of Care testing.
- Maintain HIPAA compliance and protect patient information.
