Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Topeka, KS area. The role involves direct patient care in the home or alternate sites, with a focus on assessments, care planning, and education.

The position offers mileage reimbursement and competitive hourly pay, with on-call rotation and a supportive team. Qualifications include active RN license and 3+ years in infusion therapy; BSN preferred and certifications such as CRNI/ONS/IgRN are a plus.

Qualifications

• Current RN license in good standing.
• BSN degree preferred.
• Advanced certification in specialty areas (CRNI/ONS/IgRN) preferred but not required.

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communications with care team members.
• Educate patients and caregivers to safely administer home infusion therapy.
• Evaluate home environment and identify safety interventions for prescribed care.
• Develop and evaluate plan of care with other team members.
• Manage schedule via approved platform and communicate changes promptly.
• Coordinate patient care via approved platform.
• Participate in nurse on-call rotation.
• Document all patient encounters and care activities per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of POTs and clinical documents per policy and state guidelines.
