Job summary

Soleo Health seeks a Per Diem Infusion Nurse to support infusion patients in Longmont, CO and surrounding area. You will provide direct patient care, assessments, care planning, and education in home or alternate sites per care matrices and best practices.

Requirements include active Colorado RN license, 3+ years infusion experience preferred, and flexibility for days, evenings, or weekends. Mileage reimbursement and competitive hourly pay accompany this role.

Qualifications

• Current RN license by Colorado Board of Nursing, in good standing.
• BSN degree preferred.
• Minimum 3 years nursing with infusion therapy background preferred.
• Emergency availability: nights, weekends and holidays as required.
• Experience in ICU or Cardiac Care preferred.
• Valid driver’s license with no significant restrictions.

Responsibilities

• Provide direct care for assigned patients, coordinating with care team members.
• Educate patient/care partner to safely administer home infusion therapy.
• Assess home environment for safety and suitability of care.
• Develop and evaluate plan of care with the team.
• Manage schedule via company platform and communicate changes promptly.
• Coordinate patient care through the approved platform.
• Participate in nurse on-call rotation to ensure continuous support.
• Document all encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols and standards.
• Ensure timely submission of POTs and clinical documents.
