Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Parker, CO and nearby areas. The nurse delivers direct patient care, conducts assessments, and educates patients and care partners on home infusion therapy per care plans.

Responsibilities include coordinating with the care team, documenting encounters, managing schedules, and participating in on-call coverage. Colorado RN license and infusion experience are required; driving to patient homes is expected with

Qualifications

• Current RN license in good standing (Colorado)
• BSN degree from an accredited institution preferred
• Advanced certifications such as CRNI, ONS, IgRN are preferred but not required
• Minimum 3 years nursing experience with infusion therapy preferred
• Experience in ICU and/or Cardiac Care preferred
• Proficient with Microsoft Outlook and Microsoft Office Suite; knowledge of AlayaCare, Insight, CPR+, Concur, and Paylocity helpful
• Valid state-issued driver’s license with no significant restrictions on that license
• Administrative Oversight handling workflows and compliance

Responsibilities

• Provide direct care for assigned patients, coordinating with care team
• Educate patient/care partner to safely administer home infusion therapy
• Evaluate home environment and implement safety interventions
• Develop and evaluate plan of care with the team
• Manage schedule via company platform and communicate changes
• Coordinate patient care through approved platform
• Participate in nurse on-call rotation for continuous support
• Document patient encounters and ensure regulatory compliance
• Maintain knowledge of Soleo therapies, protocols, and clinical standards
• Submit plans of treatment and required documents promptly
