Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Mayfield, MI and surrounding areas. The role involves delivering direct patient care in home settings, coordinating with the care team, and ensuring safety and compliance with policies.

Qualifications

• Current RN license by the applicable state's Board of Nursing; in good standing.
• RN degree, BSN preferred but not required.
• Minimum 3 years nursing experience; infusion therapy preferred.
• Participation in an on-call rotation, including nights/weekends if needed.
• Administrative oversight responsibilities and resource management.
• Emergency availability for rapid response as required by operations.
• Must live within 25 minutes from the Mayfield, MI area.
• Must hold a single-state Michigan RN license.
• Must drive own registered/insured vehicle; mileage reimbursement provided.

Responsibilities

• Provide direct patient care for assigned infusion patients in home or alternate sites.
• Assess, educate, and plan care with the care team and patient care partners.
• Evaluate home environment for safety and appropriateness of care.
• Develop and adjust the plan of care with the team.
• Manage scheduling through company platform and communicate changes promptly.
• Coordinate patient care using approved company platform.
• Participate in nurse on-call rotation to ensure 24/7 support.
• Document all encounters and activities per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
