Job summary

Qualifications

• Active RN license in the state of employment.
• Current BLS for healthcare professionals.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems.

Responsibilities

• Provide direct patient and family care using the nursing process.
• Coordinate Plan of Care with the Interdisciplinary Group (IDG).
• Deliver palliative and supportive care to patients and families.
• Complete documentation per policy and ensure timely admissions/discharges.
