Job summary

Chapters Health System is seeking a Registered Nurse to manage admissions across hospice services. You will educate patients and families, initiate and coordinate the Plan of Care, and perform skilled nursing interventions to maximize comfort and safety.

You will collaborate with the IDG, ensure accurate documentation, and support timely discharges while maintaining compassionate, professional care in a fast-paced environment.

Qualifications

• RN license active in the work state.
• BLS for healthcare professionals required.
• 1 year hospice or hospital nursing experience preferred.
• Experience with EMR/EHR systems.
• Mobile driver with valid license and insurance to meet visit schedules.
• Reliable transportation and ability to navigate various care settings.

Responsibilities

• Completes clinical admissions for all Company services and educates patients and families empathetically.
• Initiates Plan of Care and coordinates with Physicians; documents per policy.
• Delivers skilled nursing interventions to prevent complications and maximize comfort.
• Communicates with IDG and other health professionals; manages referrals and follow-ups.
• Represents the company at facilities; ensures timely discharge planning and SBAR updates.
• Performs duties with professionalism and complies with regulations and safety standards.
