Job summary

Alpha Omega Hospice is seeking a Per Diem Hospice Registered Nurse Case Manager (RNCM) to join our dedicated team. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey across Ellis and Dallas County.

Per Diem schedule, Monday–Friday, 8 am–4:30 pm, with after-hours on-call. We offer a comprehensive benefits package and opportunities for growth in a mission-driven organization focused on compassionate, patient-centered

Qualifications

Hold current unencumbered RN license.
Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
CPR certification if required by state.
Current driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

Empower through engagement with compassionate, expert care.
Collaborate with Clinical Director to assess eligibility for hospice services.
Nurture direct care by coordinating services while preserving patient comfort and quality of life.
Provide nursing care rooted in proven principles to ease pain and bring solace.
Educate and advocate for patients and families to create personalized care plans.
Share call duties to be there when it matters most.
