Job summary

Hospice of North Carolina is seeking a Per Diem Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team providing exceptional end-of-life care throughout the Murphy community.

You will deliver hands-on nursing care, coordinate services, and guide patients and families through every step of the hospice journey as part of an interdisciplinary team dedicated to comfort, dignity, and connection.

Qualifications

• Active RN license in good standing.
• Minimum 1 year RN experience in medical-surgical/acute care.
• Hospice experience is a plus.
• Proficient with EMR software.
• CPR certification if required by state.
• Valid driver’s license and reliable transportation.

Responsibilities

• Respond to clinical referral information with care and expertise.
• Partner with Clinical Director to assess eligibility for hospice services.
• Provide direct patient care and ensure comfort and quality of life.
• Deliver nursing care rooted in proven principles.
• Educate patients and families to create personalized care plans.
• Participate in on-call duties.
