Job summary

Hospice of North Carolina is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our nursing team delivering end‑of‑life care throughout the community. You will assess needs, coordinate services, and support patients and families through the hospice journey.

This role emphasizes compassionate care, clinical excellence, and collaboration with an interdisciplinary team. The pay is $39–$40 per hour, with a per diem schedule and opportunities for professional growth and ongoing onboarding.

Qualifications

• Hold a current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Must be computer proficient, including EMR usage.
• Current CPR certification if required by state; valid driver's license and transportation.

Responsibilities

• Empower through engagement: respond to referrals with care and expertise.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Nurture direct care as an RN Case Manager, prioritizing patient comfort and quality of life.
• Deliver nursing care with heart, using evidence-based practices.
• Educate and advocate for patients and families to participate in care plans.
• Share call duties as part of the team.
