Job summary

Crescent Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team delivering end-of-life care across York, Chester, and Lancaster counties. You will coordinate services, provide hands-on nursing care, and guide patients and families through every step of their hospice journey.

You will empower patients and families through education, collaborate with our Clinical Director, and deliver compassionate, clinically excellent care.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.
