Job summary

Frontier Hospice in Tulsa, OK is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem shifts (Mon-Fri, 8:30 am - 4:30 pm). You will provide hands-on hospice nursing, coordinate services, and guide patients and families through the hospice journey with dignity and clinical excellence.

Join our interdisciplinary team to educate families, collaborate with the Clinical Director on patient eligibility, and deliver care that respects comfort, quality of life, and human connection

Qualifications

• Hold an active, unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice a plus.
• Proficient with computers, typing, and EMR.
• Current CPR certification if required by state.
• Driver's license with reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Coordinate direct patient care as an RN Case Manager to ensure comfort and quality of life.
• Deliver nursing care rooted in proven principles and effectiveness.
• Educate patients and families to empower personalized care plans.
• Participate in on-call duties as part of team responsibilities.
