Job summary

Crescent Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our Per Diem nursing team providing end‑of‑life care across York, Chester, and Lancaster counties. You will deliver hands-on nursing care, coordinate services, and guide patients and families through the hospice journey with compassion and clinical excellence.

You’ll collaborate with an interdisciplinary team, educate families, and participate in on-call duties to ensure The Best Care Possible.

Qualifications

• Hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
• Proficient with EMR systems and basic computer tasks.

Responsibilities

• Deliver hands-on hospice nursing care.
• Coordinate services and care plans for patients and families.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide comfort, pain management, and symptom relief.
• Educate patients and families to participate in care plans.
• Assist with on-call duties as needed.
