Job summary
Altus Hospice Care is seeking a Per Diem Hospice Registered Nurse Case Manager (RNCM) in Beaumont to join our compassionate, patient‑centered team. You will deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.
As an RN Case Manager, you will collaborate with our Clinical Director, provide comfort and quality of life, educate families, and participate in on‑call coverage.
Qualifications
- Must hold current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
- Strong computer skills and EMR proficiency.
- Current CPR certification if required by state.
- Valid driver's license and reliable transportation.
Responsibilities
- Respond to clinical referral information with care and expertise.
- Collaborate with Clinical Director to assess eligibility and suitability for hospice services.
- As an RN Case Manager, provide direct nursing care to ensure comfort and quality of life.
- Deliver nursing care rooted in proven principles and techniques.
- Educate and advocate for patients and families to create personalized care plans.
- Participate in on-call duties as part of the team.
