Job summary

UnitedHealth Group is seeking a Per Diem Hospice Nurse Practitioner to deliver compassionate, patient-centered end-of-life care in home-based hospice settings. You will work with an interdisciplinary hospice team to assess needs, manage symptoms, and support patients and families through the end of life.

Key duties include communicating significant findings to the clinical team, performing face-to-face assessments, ordering appropriate tests, and coordinating referrals.

Qualifications

• Current and unencumbered Nurse Practitioner license for the state of employment.
• Current CPR certification.
• 1+ year of NP or hospice RN experience within the last 3 years.
• Knowledge of Medicare/Medicaid reimbursement and coding for services.
• Authority to practice as NP and to prescribe in the state of services.

Responsibilities

• Communicate significant findings or changes to the clinical team.
• Conduct face-to-face encounters and assess disease progression and symptom control.
• Order and review labs, imaging, and tests for palliation per patient condition.
• Make referrals to other disciplines as indicated.
• Participate in interdisciplinary team conferences.
