Amedisys, part of the Optum family, seeks a Per Diem Hospice Nurse Practitioner to deliver compassionate, patient-centered end-of-life care in home or community settings. You will assess needs, manage symptoms, and support patients and families as part of an interdisciplinary team.

The role requires NP licensure in the state, CPR, Medicare billing privileges, and the ability to prescribe. Pay ranges from $50.85 to $76.27 per hour, with comprehensive benefits and flexible per diem scheduling.

Formación

• Current and unencumbered NP license in the assigned state.
• CPR certification.
• Authorized to practice and to prescribe in the state.
• Medicare billing privileges for nurse practitioners.
• Meet state NP collaborative requirements and provide physician agreements.
• Insurance required to practice or eligible via group coverage.
• 1+ years of NP or hospice RN experience in last 3 years.
• Knowledge of Medicare/Medicaid reimbursement and coding.
• Driver's license and reliable transportation.

Responsabilidades

• Communicate significant findings to the clinical manager and team.
• Complete face-to-face encounters and assess disease changes and QoL.
• Order/review labs and diagnostics for palliation per patient plan.
• Make referrals to other disciplines as indicated.
• Actively participate in interdisciplinary team conferences.
