Job summary

UnitedHealth Group is seeking a Per Diem Hospice Nurse Practitioner to deliver compassionate, patient-centered end-of-life care in collaboration with an interdisciplinary hospice team. The role focuses on comfort, dignity, and quality of life for patients and families.

Responsibilities include assessment, symptom management, and coordinating referrals, with pay based on local labor markets and eligibility for comprehensive benefits and incentive programs.

Qualifications

• Current and unencumbered NP license in the assigned state.
• 1+ years as NP or 1 year as hospice RN in last 3 years.
• Authority to practice as NP in the assigned state.
• Authority to prescribe as NP in the assigned state.
• Must have Medicare billing privileges as NP.
• Must meet state restrictive collaborative requirements and provide agreements.
• State-required insurance for NP practice or eligibility via employer.
• Knowledge of Medicare reimbursement and coding for various care levels.
• Current CPR certification.

Responsibilities

• Communicate significant findings or changes to the clinical team.
• Complete face-to-face encounters assessing disease-related changes and quality of life.
• Order and review labs/diagnostics for palliation and treatment; document plan and review results.
• Make referrals to other disciplines as indicated by patient needs.
• Participate actively in interdisciplinary team conferences.
