Amedisys, part of the Optum family, is seeking a Per Diem Hospice Nurse Practitioner to provide compassionate, patient-centered care to individuals with life-limiting illnesses, focusing on comfort, dignity, and quality of life.

Working with an interdisciplinary hospice team, you will assess patient needs, manage symptoms, and support patients and families through end-of-life care, with a flexible schedule and per-visit compensation.

Qualificações

• Current and unencumbered NP license in the assigned state.
• 1+ year NP or hospice RN experience in last 3 years.
• Authority to practice and prescribe as NP in the state.
• Medicare billing privileges.
• Must meet state-specific collaborative requirements with physician.
• Must have required insurance to practice in state or be covered by group plan.
• Knowledge of Medicare reimbursement and coding.
• Current CPR certification.

Responsabilidades

• Complete face-to-face encounters and assess disease changes, decline in function, adherence and effectiveness of pain and symptom regimen, the patient and family coping mechanisms and quality of life.
• Communicate significant findings, problems, changes in condition or environment to the clinical manager/attending physician and other members of the interdisciplinary team.
• Order and review labs, imaging, EKGs and other diagnostic procedures for palliation and treatment of the terminal disease and document the treatment plan; review results with collaborating physician.
• Make referrals to other disciplines as indicated by the patient's needs.
• Actively participate in interdisciplinary team conferences.
