Job summary

UnitedHealth Group is seeking a Per Diem Hospice Nurse Practitioner to deliver compassionate, patient-centered end-of-life care in the home setting. You will work with an interdisciplinary hospice team to assess needs and manage symptoms, emphasizing comfort and dignity for patients and families.

Responsibilities include communicating findings to the team, conducting face-to-face encounters, ordering diagnostics, and coordinating referrals.

Qualifications

• Current and unencumbered NP license in the state served.
• Driver’s license and reliable transportation.
• Current CPR certification.
• 1+ years NP or hospice RN experience in last 3 years.
• Authority to practice as NP in the state of service.
• Authority to prescribe as NP in the state of service.
• Medicare billing privileges as NP.
• Active collaborating physician agreements per state requirements.

Responsibilities

• Communicate significant findings to the clinical manager and interdisciplinary team.
• Conduct face-to-face encounters and assess disease-related changes and quality of life.
• Order and review labs, imaging, and other diagnostics for palliation and treatment; document plans.
• Refer to other disciplines as needed.
• Participate actively in interdisciplinary team conferences.
