UnitedHealth Group’s Amedisys hospice team is seeking a Per Diem Hospice Nurse Practitioner to deliver compassionate, patient-centered end-of-life care. You will collaborate with an interdisciplinary team to assess needs, manage symptoms and support patients and families with dignity and comfort in the home setting.

The role requires NP licensure in the state, CPR, prescriptive authority, and Medicare billing privileges.

Competenze

• Current and unencumbered NP license in the state of assignment.
• Current CPR certification.
• Authority to practice and to prescribe as an NP in the service state.
• Medicare billing privileges as an NP.
• Meet state-specific collaborative requirements with an assigned physician.
• Insurance coverage by state law or employer group plan.
• 1+ year NP or hospice RN experience in last 3 years.
• Knowledge of Medicare reimbursement and coding for all service levels.
• Driver's license and reliable transportation.

Mansioni

• Communicate significant findings to the clinical manager/attending physician and interdisciplinary team.
• Complete face-to-face encounters assessing disease changes, function decline, pain/symptom regimen, coping and quality of life.
• Order and review labs, imaging and diagnostics for palliation and treatment; document plans and review results with physician.
• Make referrals to other disciplines as needed.
• Participate actively in interdisciplinary team conferences.
