UnitedHealth Group is seeking a Per diem Hospice Registered Nurse to deliver compassionate, patient-centered care in the home setting. You will assess needs, coordinate care plans, manage symptoms, educate families and collaborate with the clinical team.

Responsibilities include initial admissions assessment, evolving care plans, ongoing communication with the care team, maintaining progress records and supporting on-call needs. Competitive benefits accompany this role.

Qualifications

• Current RN license for the state(s) you are assigned to work.
• Current CPR certification is required.
• Valid driver's license, reliable transportation, and liability insurance.

Responsabilités

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care with the clinical team.
• Communicates patient progress to the clinical manager and care team.
• Maintains individualized patient care progress records per policies.
• Records pain and symptom management changes and outcomes.
