Job summary

Amedisys, part of the Optum family, is seeking a Per Diem Hospice Nurse Practitioner to deliver compassionate, patient-centered end-of-life care in patients' homes. You will work with an interdisciplinary hospice team to manage symptoms and support patients and families throughout the care journey.

The role emphasizes comfort, dignity, and quality of life, with responsibilities spanning assessment, care planning, referrals, and participation in team conferences.

Qualifications

• Current and unencumbered NP license in the state of assignment.
• Authority to practice as a nurse practitioner in the state of services.
• Authority to prescribe as a nurse practitioner in the state of services.
• Medicare billing privileges as a nurse practitioner.
• Adherence to physician-nurse practitioner collaborative requirements.
• State insurance requirements to practice or group coverage eligibility.
• CPR certification.
• 1+ year NP or hospice RN experience in last 3 years.
• Knowledge of Medicare reimbursement and coding for home/office/nursing home care.

Responsibilities

• Communicate significant findings or changes to the clinical manager and team.
• Complete face-to-face encounters and assess disease progression, function, pain, and QoL.
• Order/review labs, imaging, EKGs, and other studies for palliation and document the plan.
• Make referrals to other disciplines as needed.
• Participate in interdisciplinary team conferences.
