Amedisys, part of the Optum family, is seeking a Per Diem Hospice Nurse Practitioner to provide compassionate, patient-centered end-of-life care. You will assess needs, manage symptoms, and support patients and families within an interdisciplinary hospice team.

Responsibilities include performing face-to-face encounters, coordinating with physicians, ordering diagnostics, and referring to other disciplines to optimize comfort and quality of life.

Qualifikationen

• Active Nurse Practitioner license in the state of employment.
• At least 1 year NP experience or 1 year hospice RN within the last 3 years.
• Authority to practice and prescribe as NP in the state of services.
• Must have Medicare billing privileges.
• Must meet state NP collaborative requirements and provide physician agreements.

Aufgaben

• Complete face-to-face encounters and assess disease-related changes and quality of life.
• Communicate findings to the clinical team and attending physician.
• Order and review labs, X-rays, EKGs, and procedures; document treatment plans.
• Make referrals to other disciplines as needed.
• Participate in interdisciplinary team conferences.
