Amedisys, part of the Optum family, is hiring a Per Diem Hospice Nurse Practitioner to provide compassionate, patient-centered end-of-life care in patients' homes. You will assess needs, manage symptoms, and support patients and families with dignity and comfort.

Responsibilities include communicating with the interdisciplinary team, performing comprehensive encounters, ordering diagnostic testing, and coordinating referrals to meet patient goals.

Competenze

• Active nurse practitioner license in the state where service is provided.
• Current CPR certification.
• Authority to practice as a nurse practitioner in the state of service.
• Authority to prescribe as a nurse practitioner in the state of service.
• Medicare billing privileges as a nurse practitioner.

Mansioni

• Communicate significant findings to the clinical manager and interdisciplinary team.
• Complete face-to-face encounters and assess disease changes, function decline, and pain control.
• Order and review labs, imaging, and tests for palliation and treatment; document treatment plan.
• Refer to other disciplines as indicated by patient needs.
• Participate in interdisciplinary team conferences.
