Job summary

The Per Diem Hospice Nurse Practitioner role focuses on comfort, dignity, and quality of life for patients with life-limiting illnesses.

You will collaborate with an interdisciplinary hospice team to assess patient needs, manage symptoms, and support patients and families through end-of-life care.

Qualifications

• Current and unencumbered NP license in the state of assignment.
• 1+ years NP experience or 1 year hospice RN in last 3 years.
• Authority to practice as NP in the service state.
• Authority to prescribe as NP in the state of service.
• Medicare billing privileges as NP.
• Must meet state collaborative requirements with a collaborating physician.
• Insurance required by state law for NP practice or eligible via employer coverage.
• Knowledge of Medicare reimbursement and coding across care settings.
• Current CPR certification.

Responsibilities

• Communicate significant findings to the clinical manager and interdisciplinary team.
• Complete face to face encounters and assess disease changes, decline in function, and quality of life.
• Order/review labs, x-rays, EKGs and other procedures; document the treatment plan with the physician.
• Make referrals to other disciplines as indicated by patient needs.
• Actively participate in interdisciplinary team conferences.
