Amedisys, part of Optum family, is seeking a Per Diem Hospice Nurse Practitioner to deliver compassionate, patient-centered end-of-life care at home, focusing on comfort, dignity and quality of life. You will work within an interdisciplinary hospice team to assess needs and manage symptoms for patients and families.

Competenze

• Current and unencumbered NP license in the state of practice.
• 1+ year experience as a Nurse Practitioner or hospice RN in last 3 years.
• Authority to practice as NP in the state of service.
• Authority to prescribe as NP in the state of service.
• Medicare billing privileges as NP.
• Must meet state collaborative requirements and provide physician agreements.
• Insurance as required by state law for NP practice or group coverage.
• Knowledge of Medicare reimbursement and coding for home, office, nursing home, etc.
• CPR certification; valid driver's license, reliable transportation and liability insurance.

Mansioni

• Communicate significant findings to the clinical manager and interdisciplinary team.
• Complete face-to-face encounters assessing changes in disease, function, adherence and quality of life.
• Order and review labs, X-rays, EKGs and other diagnostics for palliation and treatment and document the plan.
• Make referrals to other disciplines as indicated by patient needs.
• Actively participate in interdisciplinary team conferences.
