Job summary

Amedisys, part of the Optum family, seeks a Per Diem Hospice Nurse Practitioner to provide compassionate, patient-centered care to individuals with life-limiting illnesses, focusing on comfort, dignity, and quality of life. You will work with an interdisciplinary hospice team to assess needs and manage symptoms in home-based settings.

This role requires an active NP license in the state, CPR, ability to prescribe and bill Medicare, and state-specific collaborative agreements.

Qualifications

• Active NP license in the state of service.
• Current CPR certification.
• Eligible to prescribe as NP in the state.
• Medicare billing privileges.
• Active collaborating physician agreements per state requirements.

Responsibilities

• Communicate significant findings to the clinical manager and interdisciplinary team.
• Complete face-to-face encounters; assess disease changes and quality of life.
• Order and review labs/imaging for palliation; document treatment plan with physician.
• Make referrals to other disciplines as indicated by patient needs.
• Participate actively in interdisciplinary team conferences.
