Job summary

Soleo Health is seeking a Full-time Infusion Nurse to support our infusion patients in the Denver, CO area. The position involves direct patient care, assessment, education, and care planning for home infusion therapy.

You will work a 40-hour week on weekdays, drive to patient homes, and collaborate with pharmacists, prescribers, and care partners. Colorado RN license required; BSN preferred.

Qualifications

• Current RN license by Colorado Board of Nursing.
• BSN degree preferred from an accredited institution.
• Experience with infusion therapy and home care is preferred.
• Emergency availability including nights/weekends as required.

Responsibilities

• Provide direct care for infusion patients, initiating and maintaining care with the care team.
• Educate patients and care partners on home infusion therapy and safety.
• Assess home environments and implement safety and care plans.
• Develop and adjust the plan of care with the team and evaluate response to therapy.
• Coordinate care and manage scheduling via approved platforms, with on-call rotation.
• Document encounters and ensure regulatory compliance.
