Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Parker, CO and surrounding area. You’ll provide direct nursing care, assessments, care planning, and education for home infusion therapy in collaboration with the care team.

The ideal candidate is an RN licensed in Colorado, with 3+ years in nursing and infusion experience, familiar with IV access devices, and able to work a flexible schedule including days and some weekends.

Qualifications

• Current RN license in Colorado
• BSN preferred
• Advanced infusion certification preferred
• Minimum 3 years nursing experience with infusion
• Experience in ICU or Cardiac Care preferred
• Proficient with Outlook and Office Suite; AlayaCare/Insight helpful
• Valid driver’s license without significant restrictions

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communications with care team
• Educate patient/care partner to safely administer home infusion therapy
• Assess home environment and plan care accordingly
• Develop plan of care with the team and evaluate response to therapy
• Manage schedule via company platform and notify changes
• Coordinate patient care via approved platform
• Participate in nurse on-call rotation for 24/7 support
• Document all encounters and care activities per policy and state guidelines
• Maintain knowledge of Soleo therapies and clinical standards
• Ensure timely submission of POTs and clinical documents
• Assist departments to complete patient documents in a timely manner
• Perform other duties as assigned
