Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Eureka, CA and surrounding areas. The nurse will provide direct home infusion care, education, and coordination with the care team.

The role requires an active CA RN license, infusion therapy experience, and availability for on-call rotations including nights and weekends. Mileage reimbursement and competitive hourly pay offered.

Qualifications

• RN license in California in good standing.
• Minimum 3 years nursing experience with infusion therapy preferred.
• Must be able to drive and travel up to 60 miles.
• On-call rotation including nights and weekends.
• Experience in ICU/Cardiac care preferred.
• Proficient with Outlook and Microsoft Office; CPR+ knowledge beneficial.

Responsibilities

• Provide direct care for assigned patients, including initiation and maintenance of infusion therapy.
• Educate patients and care partners on home infusion administration and safety.
• Evaluate the home environment for safety and prepare the plan of care.
• Coordinate care with pharmacists, prescribers, dietitians, and care partners.
• Develop and adjust the plan of care with the team and document all communications.
