Amedisys, part of the Optum family, is hiring a Per Diem Physical Therapist Assistant to deliver skilled care in patients' homes under a licensed Physical Therapist. The role focuses on improving mobility, strength, balance, and safety within the home setting, partnering with patients, caregivers, and the care team.

Responsibilities include documenting patient progress, promoting wellness, and coordinating follow-up with the admitting agency and community resources.

Qualifications

• Graduate from an accredited PTA program as CAPTE.
• Current license to practice as PTA in the assigned state.
• Current CPR certification.

Responsabilités

• Provides physical therapy services under supervision of a PT.
• Observes and reports patient response to treatment.
• Maintains documentation in medical records and coordinates care.
• Supports wellness and prevention
• Coordinates follow-up care with other providers.
• Complies with confidentiality and regulatory requirements.
