Job summary

Oak Hill is seeking a Direct Support Professional (Substitute) in Andover, CT to provide person-centered support in a residential setting. Per-Diem schedule with a minimum of 15 hours per month and compensation of $20.39/hour, with shift-based differentials.

You'll help individuals with daily living, health monitoring, meals, and community activities while maintaining documentation and following safety protocols.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to pass background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.

Responsibilities

• Support individuals in daily living activities and promote independence.
• Monitor health information and document as required; administer medications per training.
• Transport individuals to appointments and community activities.
• Prepare meals and provide mealtime support according to plans.
• Maintain documentation in the electronic health record (EHR) and follow regulatory requirements.
• Respond to emergencies and apply behavior support strategies when needed.
• Collaborate with team and participate in trainings.
